Preface To Use of Forms

I strongly recommend that these forms NOT be used without legal consultation. Every therapy practice is different. The forms should be revised to reflect your practice and procedures.

The use of office forms should be an active process as you evaluate the types of services provided, areas of vulnerability, etc. Office forms provide a valuable opportunity to check and evaluate your policies and procedures with respect to confidentiality, informed consent, emergency procedures, financial procedures, termination, hospitalization, group therapy, child therapy, custody evaluations, managed care limitations, drug treatment, use of interns, and so on.

Secondly, the office forms provide an opportunity for you and your patient to communicate about the therapy, office policies, and legal, ethical, financial and governmental requirements which will impact on the therapy. This is a chance for you to set and clarify the ground rules of the therapy and to attempt to avoid misunderstandings, which may arise in the future. It is also an opportunity for the patient to provide input about the therapy, to clarify therapy goals and if clinically appropriate, assist in selecting the therapy method. Remember informed consent must be obtained before therapy begins and throughout the treatment.
